What comes after a hospitalization?

So you’ve been hospitalized, now you are trying to figure out what comes next. When you are in the hospital after an illness or surgery, planned or not, it can be a scary situation. You’ve met with too many people to keep track of, from the nurses to the therapists to the doctors and last but not least the social workers.  They will sit down with you and a loved one and recommend a place you’d never wanted to go, a nursing home. But what they really mean is that you need a skilled nursing facility. A skilled nursing facility employs nurses, doctors, and most of all therapists to keep you on track to recover, gain strength and get needed rehabilitation services to help you on your journey back home.  The key for you now will be selecting the right facility for you and your goals. When you are selecting a skilled nursing facility make sure to keep in mind the following items:

  • Experience of staff
  • Average length of stay
  • Proven therapy record during your hospitalizations, someone might recommend post hospital rehabilitation. This is where the social work or discharge planner comes into play.
